Морфологические признаки предоставляют информацию о размерах и форме клетки. Примеры – радиус, периметр, компактность (квадрат периметра/площадь), гладкость границ, наибольшая ось сечения, симметрия, вогнутость. Текстура – это набор взаимосвязанных пикселей, которые повторяются на изображении. Текстурные (структурные) признаки предоставляют информацию о вариации интенсивности поверхности путем получения таких свойств как гладкость, шероховатость и регулярность. Фрактал – это объект, обладающий таким свойством как внутреннее единство схожести (подобия), т.е. выглядит одинаковым на разных увеличениях. При фрактальном анализе измеряется степень самоподобия. Основные свойства этого подхода – это фрактальный размер и лакунарность. Топологические свойства предоставляют информацию о структуре ткани путём вычисления пространственного распределения (взаимоотношения) клеток.

Самым распространенным методом анализа интенсивности изображений является использование цветовых гистограмм. Они не предоставляют информацию о пространственном распределении пикселей. Пример такого анализа – определение оптической плотности пиксела, путем конвертирования значения уровня серого в соответствующее значение оптической плотности. Могут определяться значения пикселей отдельного цветового канала или взаимосвязь между значениями пикселей различных каналов [1].

Публикаций по теме лимфангиогенеза и лимфогенного метастазирования опухолей щитовидной железы немного [2-26]. Движущая сила научного продвижения этой области – недавнее появление лимфатических маркеров; среди них D2-40, который применялся в нашем исследовании. Антитело D2-40 первоначально реагирует с O-связанным гликопротеином, экспрессирующемся на лимфатическом эндотелии, эмбриональном яичке и опухолях яичек.

На сегодняшний день считается, что основные биохимические факторы развития лимфангиогенеза любой локализации – сосудистые эндотелиальные факторы роста (C, D, А, в порядке значения). Неизвестны механизмы увеличения VEGF (фактора роста эндотелия сосудов), и природа молекул, выполняющих активацию существующего лимфатического эндотелия, вызывающего увеличение размера опухоли с последующей продукцией митогенных и хемотаксических факторов или альтераций лимфатического эндотелия.

Hall [27] и сотрудники, 2003, были первыми, кто исследовал морфологические особенности опухолевой лимфатической сети у больных, имеющих папиллярные карциномы щитовидной железы. В исследовании, используя LYVE-1 (Lymphatic vessel endothelial hyaluronan receptor) как маркера, сетки Chalkleу (см. рис. 3-6) для подсчета сосудов, высокая ПЛК (плотность лимфатических капилляров) была связана с наличием региональных метастазов лимфатического узла. Однако наличие внутриопухолевого лимфангиогенеза не было значительным предиктором рецидива опухоли (P = 0.42). Интересно, что эксперимент Cheong [28, 29] и соавторов (2010) не показал значительных различий в плотности лимфатических и кровеносных капилляров сосочковых карцином щитовидной железы и микрокарцином (<1 см в размере). Однако более высокая ПЛК отмечалась у больных старше 45 лет (более четко в группе крупных карцином) и имеющих мультицентричность с экстратиреоидным распространением. Мультицентричность была определена как наличие дополнительных фокусов опухоли, состоящих из нескольких несмежных с первичной опухолью участков. Экстратиреоидное распространение было определено как прорастание опухоли вне капсулы, окружающей мягкие ткани железы.

Классическим морфометрическим параметром оценки сети лимфатических капилляров является их площадь, прокрашенная диагностическим маркером [30-37]. В нашем исследовании применены новые параметры изображений. Как оказалось, доля мелких и крупных объектов характеризует морфологию сети лимфатических сосудов более подробно, чем просто площадь прокрашенного, и выявляет свою прогностическую ценность. Новые параметры изображений позволяют классифицировать морфологию ангиогенеза и выделять подгруппы морфологических паттернов.

Оценка степени активности ангиогенеза на гистологическом препарате

Ангиогенез можно рассматривать как на уровне организма пациента, так и на уровнях системы органов, индивидуального органа, тканей и отдельных клеток. Ангиогенез на микропрепарате оценивается либо глазом врача-патолога, масштабной сеткой (graticule) и с помощью стереологических методов, или с помощью программного обеспечения (Goddard и соавт., 2002) [38-40]. Как правило, обработка изображений, по которому оценивается степень ангиoгенеза, проходит в несколько этапов [41].

Первый метод количественной оценки ангиогенеза на примере неоваскуляризации опухолей мозга предложил Brem (1972) [42].

Изначально, количественной оценкой ангиогенеза на гистологическом срезе органа являлась площадь сосудов (vessel area), окрашенная красителем, [43] или иммуноцитохимическим методом. Поиск надежных биомаркеров ангиогенеза продолжается и сегодня [44-48], и, вместе с этим направлением, предлагаются новые критерии оценки изображений ангиогенеза, полученные в результате сложного процесса приготовления микропрепарата изучаемой ткани. На сегодняшний день наибольшее распространение среди параметров оценки ангиогенеза получил показатель плотности микрососудов (microvessel density), определяемой в точках максимальной васкуляризации («горячих точках» – hot spots). Этот подход считается «золотым стандартом» оценки активности ангиогенеза в тканях опухолей [39, 47, 49-52]. Его авторы, Weidner с сотрудниками, в начале девяностых годов разработали и применили эту методику для исследования карцином молочной железы и простаты [42].

Ангиогенез (измеренный как плотность микрососудов опухоли) коррелирует с поведением опухоли (см. таб. 3 и рис. 6-9). Факты свидетельствуют, что интенсивность ангиогенеза коррелирует с развитием метастазов, плохим прогнозом, в том числе уменьшением срока выживаемости у пациентов, страдающих раком молочной железы, мочевого пузыря, желудка (Turner и соавт., 2003) [53]. Что касается исследования ангиогенной активности в тканях эпителиального рака яичников, то в этой области сообщаются противоречивые сведения о значении плотности микрососудов как предсказательном факторе клинического прогноза и критерии дифференциальной диагностики. Bamberger&Perret, 2002, пришли к заключению, что экспрессия VEGF и анализ плотности микрососудов для оценки васкуляризации эпителиальной карциномы яичников не являются настолько полезными для предсказания прогноза, как при исследовании карциномы молочной железы, и что не наблюдается четкой корреляции между значением плотности микрососудов и возрастом, степенью дифференциации опухоли, её размерами, асцитом, ростом опухоли, причем наблюдаются колебания этой зависимости для различных гистологических подтипов рака яичников [54]. Дальнейшие исследования также выявили аналогичные противоречия [47]. Можно предположить, что разные выводы исследований обусловлены разнородностью генотипов обследуемых пациентов в ракурсе экспрессии сосудистых биомаркеров, либо гетерогенностью микропрепаратов. О недостатках и ограничениях метода определения плотности микрососудов в «горячих точках» высказывались Weidner [55] и Brown [44]. Последний, на основе личных наблюдений и анализа исследований других ученых, пришел к выводу, что этот метод не может применяться для точной оценки активности ангиогенеза в широкомасштабных клинических исследованиях. Таким образом, недостатки «золотого стандарта» оценки процессов ангиогенеза заставляет искать и оценивать новые критерии анализа изображений микропрепаратов окрашенных капилляров.

Альтернативным подходом к количественной оценке ангиогенеза является подсчет микрососудов в случайно (рандомизированно) выбранных областях изображений или в специфически определенных регионах, таких как края опухоли. Дают ли оба названные методы одинаковую биологическую информацию и лучше ли они других методов – этот вопрос открыт для дискуссии (Kim и соавт., 2003 in «An original approach…»). Помимо самой плотности микрососудов, может вычисляться и среднее значение пропорции поверхности микрососудов (mean microvessel surface proportions), также фракция площади сосудов (vascular area fraction), абсолютное число микрососудов и их периметр, длины и площади определенных категорий капилляров [40, 56, 57].

Когда вычисляется плотность микрососудов, отдельный объект, прокрашенный, например, маркером CD34, регистрируется как один сосуд. Считается, что по своей природе иммунохимическое окрашивание сосудов не является идеальным для диагностических целей, так как при такой окраске прилегающие сосуды и даже группа сосудов могут быть распознаны как один объект (Goddard и соавт., 2002) [39, 52].

После бинаризации изображений, плотность микрососудов оценивается путем подсчета общего числа белых пикселей на определенном поле. Такие показатели как среднее число концевых точек капилляров (vessel end points), узловых точек разветвления на каждом изображении (vessel branch points/nodes), средняя общая длина сосудов для данного изображений в пикселях вычисляется после скелетонизации изображений (Аmano и соавт., Japan, 2007) [58].

Объектом измерения ангиогенеза программы AngioQuant (программного обеспечения бесплатно доступного для научно-исследовательских целей по адресу www.сs.tut.fi) является сеть соединенных трубчатых (тубулярных) структур капилляров. Это программное обеспечение позволяет определять длину и размеры этих тубулярных комплексов, также как и число соединений (точек ветвления) в самом комплексе (Antti Niemisto, Finland, 2005) [59]. Исследователи, разработавшие это программное обеспечение, пришли к выводу, что распределение длин тубулярных комплексов в экспериментальном ангиогенезе подчиняется закону степени (power law).

В своих статьях об измерении ангиогенеза посредством анализа изображений препаратов мембраны эмбриона цыпленка и аортального кольца крысы, Blacher и соавт., 2001, 2005 [60, 61], упоминают такие характеристики оценки ангиогенеза как плотность длины сосудов (vessel length density), в частности радиально расположенных капилляров, размеры фракций капилляров (fractial dimension), средний диаметр капилляра.

Другие подходы к оценке изображений ангиогенеза – это анализ цветов полученного изображений, цветовая сегментация изображений (сolor segmentation), сравнение цветов пикселей (pixel colour comparisson), анализ текстуры изображений гистологического препарата (Rodriguez и соавт., 2002, Laitakari, 2003) [62, 63]. Последний метод характеризует энтропию, корреляцию и контраст изображений.

Интересный подход к анализу гистологического препарата ангиогенеза – применение фрактального и синтаксического анализа (fractal & syntactic analysis) c применением спектра декскрипторов, многогранно описывающих объекты изображений (Grizzi и соавт., 2005) [64].


Окулярные сетки применяются в гистологии и смежных науках со времен изобретения микроскопа. В настоящее время «золотой» стандарт морфометрической оценки микропрепарата ангиогенеза это применение сетки-гратикулы Сhalkley, предложеной в 1943 году. Широкое применение микроскопической техники в различных областях науки и техники обусловило спрос на методы измерения микрообъектов. Сейчас коммерчески доступны разнообразные измерительные инструменты для оптического оборудования. Однако патология и гистопатология уже давно стали «цифровыми» и математическими. Программное обеспечение развилось от примитивных медленно вычисляющих арифметических оболочек до сложно организованных многофункциональных, удобных для пользователя вычислительных систем, работающих на разных платформах и позволяющих реализовывать те идеи и цели, которые сравнительно недавно были невозможны по причине недостатков как самих персональных ЭВМ, так и по причине ограничений возможностей самих языков программирования и сред разработки, в которых программное обеспечение создается. Накопленный опыт применения объектно-ориентированнного программирования и открытые библиотеки кода, а также возможности экстрагировать сам код из элементов написанной программы, инструменты для трансляции кода одного языка в другой, а также его коррекции и автоматической генерации позволяют разработчикам быстрее осваивать профессию и разрабатывать программное обеспечение.

На сегодняшний день учёным доступно как бесплатно, так и коммерчески, множество пакетов программных оболочек для обработки изображений и морфометрии. Основные недостатки современного программного обеспечения:

  1. сложность в использовании, особенно для лиц, не имеющих достаточного опыта пользования персональным компьютером, причиной этой проблемы может быть непонятность инструкции и дефекты дизайна и функциональной организации интерфейса;
  2. большой объем программы и высокие требования к системе и платформе, на которой программное обеспечение запускается;
  3. невозможность беспрепятственнного запуска одновременно нескольких программ с целью быстрой обработки изображений, также сбои в функционировании при одновременной работе с другими программами;
  4. недостаток визуализации обрабатываемых программой данных, когда на выходе получаются только числа без изображения решаемой проблемы;
  5. неудобства, связанные с экстракцией, организацией и хранением параметров полученных после обработки изображений.

В заключение мы решили перечислить области науки, где обработка изображений может применяться:

  1. анализ изображений микробиологических посевов с окружающей среды (водоемов, грязных рук, воздуха операционных и т.п.) [252-258];
  2. для изучения прионных болезней [259, 260];
  3. для изучения морфологии грибов, хламидий, уреоплазм, микобактерий, там, где имеет прогностическое или другое клиническое значение количество организмов и их морфологическая характеристика [261-272];
  4. анализ тканей для изучения вирусного канцерогенеза (вирусов герпеса, Эбштайн-Барр, папиллома-вируса) [273-283];
  5. анализ изображений сигналов гибридизации in situ [284];
  6. анализ изображений эндоскопии (кишечника, пищевода, желудка, трахеи, глазного дна, шейки матки, мочевого пузыря, барабанной перепонки) [285-313];
  7. обработка изображений в репродуктивной медицине (яйцеклеток) [314, 315];
  8. изучения микро- и макромицетов и спорообразующих микроорганизмов [272];
  9. для подсчета микроорганизмов при воздействии на микробную культуру бактерицидных и бактериостатических веществ;
